Forms Necessary for Medications in School
- Medication Administration Request Form - for all medications in school
*If your child needs to take medication while at school, the parent must bring in the medication to the nurse. The medication must be in the original container. Please check expiration dates. A copy of the prescription order and dosing schedule is needed for prescription medication.
- Self-Medication Request Form - only for life-threatening conditions

*Please note that prescription medications require the prescription from the doctor AND need to be in the original container with the pharmacy label/instructions
